Qualifications:
Service delivery is expected be provided with appropriate professional license within scope of services when applicable.
Clean, valid NYS Driver's License.
Fingerprinting, criminal record check, and all necessary clearances from the Justice Center.
JOB FUNCTION:
Provide Services to Children and youth under 21 years old.
Offer community based support and services to children in non-institutionalized settings that enable them to remain at home and in the community.
Offer participants independent choices among an array of service options and providers.
These services include:
1.
Other Licensed Practitioner, 2.
Crisis Intervention.
Responsible for all documentation including assessments, service plans, progress notes, authorizations and other needed information as per program policy and procedures.
Services will be provided w/in your scope of practice so all services may not apply.
